Digital Form Changes for West Midlands

This document outlines the digital form changes for Cadent, aiming to enhance efficiency and user experience. These changes will be rolled out on the 1st October 2025.

CG009 - DAF/DR4

Changes were requested to the CG009 - DAF/DR4 to simplify the user experience and reduce the end users need to correct where incorrect matrix numbers are chosen.

Removing the Engineer Authorisation section as this is now managed external to the form.

Section 1 - Request, the user will now choose the type of request as opposed to a matrix.

CG011 - Purge & Relight

In Arrival Information, a new question ‘’Has an SSRA been completed before commencing work?’. This is a simple checkbox to confirm a SSRA has been completed before commencing the Purge & Relight.

The form Cannot be completed without confirming this statement.

CG015 - Mains Test

The changes to the CG015 - Mains Test have been implemented to support the track sheet, this is inquisition to be rolled out post 1st October 2025.

The Length has been updated to Test Length (M), and a new field called Abandonment (M).

CG018 - Service Details

The changes to the CG018 - Service Card have been implemented to allow users to easily request a missing property, this allows users to state if a property is missing from the list.
