Guide to Completing: CG012 - Reinstatement Measure

        General Details

        Tip - After completing each section Save Draft, so you can come back to it or if there is a connectivity issue you will not lose everything.

        • Scheme Number - Select the scheme from the drop-down menu.
        • Street - Select the street that you will be working.
        • Permit Number - Select relevant Permit Number 
        • Permit Planned Start and End Date - This will automatically populate from the permit number selected.
        • Status - This will automatically populate to ‘In Progress’ if any other option displays, select another permit or check with your site manager.
        • Supplier - Select the appropriate option as to who will be doing the reinstatement.
        • Workstream - Will default to ‘Replacement’ but you can update this
        • Reinstatement Type - You can choose 'Reinstatement', ‘Spoil’ or ‘Support’ and this will change the next fields to be completed.
        • Is this the final registration for this permit - If you know this is the last set of holes for the street select ‘Yes’ if not then select ‘No’.

        Reinstatement Details

        This is a repeatable section, each of these are for one hole.  If you need more holes or images to add, click the + button at the bottom of each section.

        • Location - Select nearest/suitable location to hole.
        • Private - Yes, or No?
        1. Yes - This will give you a property drop down selection 
        2. No - This will give you a free text Property Number/Name/Street to enter any details to identify the location of the hole ie “on corner of, outside of, at a junction of.” etc.  If no access, you can change the number.
        • Date Measurements Taken - A calendar will display to enter the date, once the date has been selected you will then enter the time.

          Once you have entered the date and time, if this is outside of the dates set with in the permit you will be presented with below message.  Check you are working on the correct permit before

        • Road Category - Will automatically select from the permit in the future but will go to ‘N/A’ but you can select any option and can be network specific.
        • Excavation Type - This is optional and network specific.
        • Interim/Permanent - Choose if this is a temporary fill and needs to be dug up again or a permanent fill.
        • Trench - Select Yes if over 10m.
        • Hole Length/Width/Depth - enter measurements.(Depth measurement at its deepest point) this field is numerical only and below measurements to be used;
        • a.    Length = Meters
          b.    Width = Meters
          c.     Depth = Millimetres
        • Is back fill required - Select Yes or No
          • If Yes - Back fill Length and Width will appear.
          • If No - Move on to the next question.
        • Surface Material - Enter the suitable surface material.
        • Additional Item Material - Any additional material needed, eg kerb.
          1. This will open new fields for further information such as size, amount etc

        • Photos - Upload a clear image of the excavation, including the edge to compare material needed.  You can save the image to the device before uploading.
        • Coordinates - Click on the map button and this will drop a pin to within 3 metres of your  location, you can move the pin to a more accurate location, if necessary.
        • Additional Comments - Enter any supportive comments here such as why work was not completed, any access issues such as whether the occupant needs access to their drive at a certain time, excavation behind locked gates, materials located elsewhere etc.

        If you need to add extra details press the + button, if pressed in error where it says the box number click the X on the right side to remove.

        Tip - Remember to save to draft, to save progress so far

        To submit, save to Create Reinstatement Complete.

        Spoil Details

        This is a repeatable section, if you need to add additional information or images to add, click the + button at the bottom.

        • Location - Free text box, enter property number or description of location.
        • Public/Private - Select if public or private land.
        • Estimated Tonnes - Enter estimated volume needed in tonnes.
        • Ready Date and Time - This is when the location will be ready.
        • Photos - Upload an image of the work, you can save the image to your device then upload.
        • Comments - Enter any comments.

        If you need to add extra details press the + button, if pressed in error where it says the box number click the X on the right side to remove.

        Tip - Remember to save to draft, to save progress so far.
        To submit, save to Create Reinstatement Complete.

        Support

        This is a repeatable section, if you need to add additional information or images to add, click the + button at the bottom.

        • Location - Free text box, enter property number or description of location.
        • Public/Private - Select if public or private land.
        • Detailed Requirements - Enter what support is needed/required.
        • Ready Date and Time - This is when the location will be ready.
        • Photos - Upload an image of the work, you can save an image to your device then upload.
        • Comments - Enter any comments.

        To submit, save to Create Reinstatement Complete.

        Completion out in the field.

        Open the Reinstatement Complete submission once it has been assigned to you.

        Now you can begin to capture what there is to be completed.  The form is already completed with the measures done previously, you will just need to fill in the sections for that day.

        If the permit is incorrect, it can be updated to the correct one.  Most fields will appear locked, and no changes can be made to these sections.

        Some mandatory fields you will need to check are;

        Reinstatement Completion Date/Time - Update when completed.

        Request (Gas On) Date/Time - Update when gas is back on.

        Interim/Permanent - If it has changed from Interim to Permanent this can be updated.

        Hole Length/Width/Depth - This will be filled out with what the actual measurements were for the backfill.

        Surface material - This can be changed if the incorrect material was chosen.

        Photo - Add an image of the completed work, you can save the image to your device then upload.

        Trench Co-Ordinates - Add the current location of the hole.

        Additional Comments - Add comments such as any descriptions, complications, reasons not completed etc.

        Once updated, submit and save.
