Scheme Details

The scheme details at the beginning of the form is used to identify the works associated with the form being completed.

Anything relating to an entity within Ctrl-Hub itself in the form's completion will then associate the form submission to said entity's submission area.

*Note* Any label in bold with a red asterisk is mandatory for completion.Upon selecting the Scheme relevant to the works from the drop-down menu, the Street drop-down menu options will fill with relevant addresses to the Scheme. When the street is selected the Town/City field will automatically populate with the relevant address. Enter the Eastings & Northings, there is a map icon, to open Google maps if necessary and a Cross to clear the selected Eastings & Northings. Drop-down menus also give options for Location and Engineer's Name from the database. Enter your EUS number and lastly the Date(s) should be entered to give a time stamp of the installation and commission dates.

Within the majority of forms dates are needed to ascertain when the form is completed or when the action discussed was performed or planned for. Usually this would appear as a date box, with drop-down options for Month/Day/Year.

Valve Details

The questions require drop-down selections and text inputs to gather information about the valve and has attachment and comments boxes for the upload of files to aid the submission.

The rest of the form is similar drop-down menus relating to the valve specifications.
