Guide to Completing: CG006 - Customer Impact Assessment

General Details

The general details at the beginning of each form is used to identify the works associated with the form being completed.

Anything relating to an entity within Ctrl-Hub itself in the form's completion will then associate the form submission to said entity's submission area.

*Note* Any label in bold with a red asterisk is mandatory for completion.

Upon selecting the Scheme relevant to the works from the drop-down menu, the  Street drop-down menu options will fill with relevant addresses to the Scheme and will become accessible. 

The ' Scheme Address' field will automatically populate based on the selected street. The Date should be entered to give a proposed start date, and then finally, the duration of the Scheme.

Within the majority of forms dates are needed to ascertain when the form is completed or when the action discussed was performed or planned for. Usually this would appear as a date box, with drop-down options for Month/Day/Year.

Navigation

Each form is separated into sections for ease of access to users. If, for example, you are only required to access a small part of a form for authorisation, or any other reason you can use the section navigation menu on the left-hand side of the form.

*Note* The example above doesn't showcase navigation so well, as all of the sections are within view. However, in longer forms with long sections, this menu will save a lot of time!

Form Answers

In CG006 - Customer Impact Assessment each question is laid out a similar way. Questions will be laid out at the top of each row and in this case is followed by a drop-down menu to answer.

The comments box below this may or may not be mandatory, and is for any additional notes that evidence answers, or include additional information that may be required about the drop-down option selected.

This form will add up all of the impacts/disruptions measured with the answers throughout the form's completion. At the end of completing the form, you are given a total number for the amount of everything selected.

Like most form submissions within Ctrl-Hub, the information gathered in the submission will feed back to other areas of the organisation, keeping everybody in the loop.
